Welcome. Most people call me Nkem. Coming to a new psychiatric provider can feel intimidating, especially if you have been let down by previous care. This page walks through exactly what your first appointment and follow-ups look like — so you know what to bring, what we cover, and what happens next. No surprises.

Call (775) 902-8300 or email scheduling.nv@mindfulsupportservices.com. Our administrative partner Mindful Therapy Group handles intake, insurance verification, and scheduling. They will confirm your insurance benefits, walk you through self-pay options if you are not using insurance, and book your first available window.
You will receive intake forms to complete in advance — symptom history, medical history, current medications, family history, treatment goals, and consent forms. Completing these before your visit means we spend your appointment time on you, not on paperwork. Set aside thirty to forty-five minutes for this.
If you have prior psychiatric records, bring them. If you have a list of medications you have tried (with doses, durations, and responses if you remember them), bring that. If your previous provider's notes are accessible to you, those help. None of this is required — we can work from your memory if records are not available — but anything you bring adds to the picture.
Our office is at 8215 S Eastern Ave Suite 109, Las Vegas NV 89123. In-person appointments at the office are held on Tuesdays. Parking is on-site. Plan to arrive five to ten minutes early so any last-minute administrative items can be handled before your appointment time begins.
This is the comprehensive first visit. We go through your symptoms, your history, your goals, and the medications you have tried. We discuss the diagnostic picture as it becomes clearer. We talk through treatment options together. By the end of the visit, you understand where I think you are, why, and what the path forward looks like.
Most patients return in two to four weeks for the first follow-up, then at intervals appropriate to the stage of treatment. Follow-ups are thirty or sixty minutes depending on what the work requires. We adjust the cadence as you progress.
Your first appointment begins with a brief introduction and an honest conversation about how I practice. I am clear about the boundaries that protect our work together, so there are no misunderstandings later: how cancellations and late fees work, why refills are handled through your follow-ups, and why I keep texting and email limited so nothing important gets lost between visits. We build a safety plan together. Most of the hour is spent understanding what brought you here, and how I can be of service to you.
Before you walk in, I want you to know that I truly care. I am not perfect, and I will not pretend to be. I will do my best, and I ask for honest collaboration and communication without fear. My kindness is not weakness. I hold firm, fair boundaries that protect the integrity of this practice and the trust that makes good care possible.
Telehealth or in-person. In-person visits at the Las Vegas office are held on Tuesdays. Telehealth may be available on other days depending on your needs, treatment plan, and Nevada telehealth eligibility. For most new patients, the initial evaluation is in-person; many follow-ups work well by telehealth. We discuss which fits you during scheduling.
Insurance verification. Our administrative team will confirm your specific plan and benefits before your first appointment. If you are using self-pay, the rates are listed on the pricing page. We do not surprise you with charges after the fact.
Medication refills. Refills are coordinated through your scheduled appointments and your pharmacy. If you need a refill before your next appointment, call (725) 215-1037 so we can review your case appropriately. We do not refill medications by web form for safety reasons.
Cancellations. If you need to reschedule, please give us at least forty-eight hours notice. Our administrative team sends appointment reminders ahead of time, so you have what you need to plan around it. Late cancellations and missed appointments are billed at $150.
Crisis. Evolving Minds Psychiatry is not an emergency or crisis service. If you are in immediate danger, call 911 or call or text 988, or go to the nearest emergency department.
